01If I feel better on day three, can I stop?
No. Finish the written days. A quieter day is not a culture. Stopping early is how a labelled anaerobic course, or a seven-day vaginal course, gets cut in half at home. The usual oral anaerobic stretch is 7 to 10 days after an IV start. Bone, joint, lung, and endocardium infections may run longer.
The half-life is about eight hours. That clock is about drug still in the body after a swallow, not permission to quit on day three. Remaining time after the last tablet lives on how long Flagyl lasts.
02Does a meal delay the start?
A meal can sit under the tablet if nausea is loud. Food is not a timer on when the infection eases. You may take regular metronidazole with food. The labelled peak window is still one to two hours after the dose. A 500 mg swallow peaked near 12 mcg/mL in that work.
The meal question is unpacked on Flagyl with food. Do not skip a dose to wait for an empty stomach unless the written line is an extended-release tablet, which is taken one hour before or two hours after a meal.
03When is the tablet actually in the blood?
Peak plasma concentrations sit between one and two hours after an oral dose. Metronidazole is well absorbed. Disposition after a swallow is similar to an intravenous dose. A 500 mg swallow produced about 12 mcg/mL in the labelled work. A 2,000 mg swallow produced about 40 mcg/mL. A 250 mg swallow produced about 6 mcg/mL.
Plasma concentrations stay proportional to the administered dose. Studies found no significant bioavailability gap between males and females; weight still left males with generally lower levels. Those numbers describe drug in plasma. They do not tell you the hour discharge stops. The metronidazole shelf bins the 500 mg tablet this site talks about.
04Does a one-day 2 g swallow work that afternoon?
The 2 g trichomoniasis regimen is one written day - a single dose, or 1 g twice on that same day. Peak blood is still in the one-to-two-hour window, near 40 mcg/mL after 2,000 mg. Symptom change can lag that peak. The partner still needs a plan so the organism does not come back.
The seven-day 250 mg three-times-daily line is the other labelled trichomoniasis course. The writer picks one calendar. How those two options are chosen sits on how long a Flagyl course is.
05How soon do symptoms usually move?
They may ease over days. The US Flagyl label does not give a clock like "feel better at 48 hours." Trichomoniasis, bacterial vaginosis, amebiasis, and an anaerobic wound are different infections. Each has its own written days. A quieter afternoon after the peak is still not a culture.
If day two still feels the same, that is not a signal to add a second 2 g swallow at home. Call the clinician who named the indication. Resistance can develop; stacking extra tablets is not how the label handles a slow start.
06What if a week later nothing has changed?
Then the next step is the clinician who wrote the course, not a second bottle from a friend. Repeat trichomoniasis treatment, when it is needed, waits four to six weeks and reconfirms the organism by smear or culture. Total and differential leukocyte counts belong before and after that re-treatment. A leftover 500 mg strip does not restart the calendar. The partner plan is checked again if reinfection is why the first days failed. Do not add a home 2 g swallow on day eight.
