How much pregabalin does the US label start?

150 mg a day. That is the usual adult start on the US label for the pain uses, given as 75 mg twice a day or 50 mg three times a day when creatinine clearance is at least 60 mL/min. This shelf talks about the 75 mg capsule. The next step, when it happens, is often 300 mg a day within a week. Kidneys that already run slow need a smaller total from day one.

01What if creatinine clearance is already low?

Cut the daily total. Pregabalin leaves the body through the kidney. The label has a creatinine-clearance table. One labelled example: a postherpetic neuralgia start of 150 mg a day at clearance of 60 mL/min or more becomes 75 mg a day at a clearance of 50 mL/min, still split into two or three doses.

Hemodialysis needs a reduced daily total plus a supplemental dose after a 4-hour run. Every row of that table is not copied here. The clinician who writes the first bottle should have a recent clearance, not a guess.

02Where does 600 mg a day actually sit?

Adjunctive treatment of partial-onset seizures in adults can go to 600 mg a day, split into two or three doses, after a start no greater than 150 mg a day. Nerve pain after spinal cord injury is written as 150 to 600 mg a day, starting at 75 mg twice a day.

Those ceilings are indication-specific. Copying a 600 mg seizure total onto a fibromyalgia start is not what the label writes. How soon pregabalin works is about the first week, not about jumping to the highest number on the file.

03How does postherpetic neuralgia start?

Begin at 75 mg twice a day or 50 mg three times a day (150 mg/day) when clearance is at least 60 mL/min. The total may rise to 300 mg a day within one week. The labelled range after that is 75 to 150 mg twice a day, or 50 to 100 mg three times a day (150 to 300 mg/day).

People who need more, and who tolerate 300 mg a day, may go as high as 600 mg a day on this use. That is a later step, not the first bottle. Reduced kidney function changes every one of those numbers.

04How does diabetic nerve pain start on the label?

Begin at 50 mg three times a day (150 mg/day). The daily total may rise to 300 mg within one week if that start is tolerated. The maximum recommended dose for this use is 100 mg three times a day (300 mg/day) when clearance is at least 60 mL/min.

75 mg twice a day is the other way to reach that same 150 mg start on related pain uses. The clinician picks the split. A DPN start does not move to 600 mg. That higher ceiling belongs to other labelled uses. The pregabalin shelf uses that 75 mg piece.

05How does fibromyalgia start?

Begin at 75 mg twice a day (150 mg/day). The total may rise to 150 mg twice a day (300 mg/day) within one week. People who still do not have enough benefit on 300 mg a day may go to 225 mg twice a day (450 mg/day).

600 mg a day was studied in fibromyalgia. The label says there is no evidence that dose adds benefit, and it was less well tolerated. Treatment above 450 mg a day is not recommended for this use. That is a different ceiling than postherpetic neuralgia.

06Which form carries those milligrams?

Capsules come in 25, 50, 75, 100, 150, 200, 225, and 300 mg. The oral solution is 20 mg/mL. Prescriptions for the solution are written in milligrams. The pharmacist converts 150 mg into 7.5 mL.

Do not change the daily total without talking to the clinician. Do not stop suddenly. The label wants a taper over a minimum of one week. Two capsules at once is not how a missed swallow is repaired. Swallow can be with food or without. Store the bottle at 59 F to 86 F (15 C to 30 C) in the original package.

Is 75 mg the whole daily total?

Usually no. 75 mg twice a day is a common 150 mg daily start. The clinician may instead use 50 mg three times a day for the same total.

Can I go straight to 600 mg because a forum said that is the max?

No. 600 mg is a ceiling on some uses, not the start, and fibromyalgia is not recommended above 450 mg a day. The first total is 150 mg a day for most adults with working kidneys.

The labelled example for a 150 mg postherpetic start becomes 75 mg a day at 50 mL/min. Bring the number to the clinician. Do not keep the usual map.
