01What if an erection lasts more than 4 hours after the fill?
Rare reports exist of erections lasting greater than 4 hours and priapism greater than 6 hours for this class. An erection lasting greater than 4 hours, painful or not, needs emergency care. Caution sits on sickle cell anemia, multiple myeloma, leukemia, and anatomical deformation of the penis such as Peyronie's disease.
02Can a local pharmacy fill the same day?
Once the clinician sends the order, a local pharmacy can fill the strengths it has on the shelf. Same-day stock is that pharmacy's inventory. A mail-order house fills the same written order and ships when its own stock moves.
The pharmacist who hands over the bottle can still ask about nitrates, alpha-blockers, and the last dose. CIALIS for BPH is not recommended with an alpha-blocker. For erectile dysfunction, the person should already be stable on the alpha-blocker, and CIALIS starts at the lowest recommended dose.
Food does not set the swallow. Sexual stimulation is still required to start the local release of nitric oxide. The pharmacist can also ask about alcohol: 5 glasses of wine or 5 shots of whiskey sit on the patient-information example for too much alcohol with this tablet.
03Does a urologist have to write tadalafil?
A primary-care clinician who can see the chart can write tadalafil. The US Cialis label is a prescription product. A referral happens when that first clinician wants a second look.
Before the pen moves, the chart has to show the nitrate list is empty. CIALIS is not recommended after a myocardial infarction in the last 90 days, after unstable angina or angina during intercourse, after New York Heart Association Class 2 or greater heart failure in the last 6 months, with uncontrolled arrhythmias, with blood pressure under 90/50 mm Hg, with uncontrolled hypertension, or after a stroke in the last 6 months. Those groups were left out of the safety and efficacy trials.
Nitrates on that stop include nitroglycerin in tablets, sprays, ointments, pastes, or patches, and isosorbide dinitrate or isosorbide mononitrate. Recreational drugs called poppers also carry nitrates, such as amyl nitrite and butyl nitrite. If a nitrate later looks medically necessary in a life-threatening situation, at least 48 hours should elapse after the last CIALIS dose, and the nitrate still belongs under close monitoring.
04What if the liver is impaired on that same order?
Child-Pugh A or B keeps as-needed CIALIS at 10 mg or less once per day. Severe hepatic impairment, Child-Pugh C, is not recommended. Once-daily CIALIS has not been extensively evaluated when the liver is impaired.
05What if ketoconazole or ritonavir is already on the chart?
Ketoconazole or ritonavir on an as-needed CIALIS order drops the maximum recommended dose to 10 mg, not more than once every 72 hours. For once-daily CIALIS with those same inhibitors, the maximum recommended dose is 2.5 mg.
06How does FDA BeSafeRx check an online pharmacy?
FDA's BeSafeRx list is four checks. The site requires a prescription from a licensed clinician. It posts a physical US address and a US phone number. A licensed pharmacist is there to answer questions. The pharmacy holds a state board license in the state where it operates and, if different, in the state where you live.
FDA's locator on FDA.gov is how you look that license up. If the pharmacy is not in that database, FDA says not to use it. A listing still needs the other three checks. FDA warning letters name sites that offer prescription drugs without a prescription, without adequate directions, or without the required risk language.
FDA also tells people not to buy from sites that sit outside the United States. A shop that skips the written order, hides its address, or will not name a licensed pharmacist fails that list even if the page looks like a US drugstore.
07If the visit is on a screen, where does the tablet come from?
A video visit still ends at the same two steps: a licensed clinician writes the tablet, then a pharmacy fills that order. Mail-order and a local pickup both wait on that written order.
The clinician who writes has to be licensed where the person sits. The order can name Cialis or tadalafil. The strengths on a US Cialis order are 2.5 mg, 5 mg, 10 mg, and 20 mg. The pharmacy fills the line that arrived, not a strength the website invented.
As-needed erectile dysfunction on a US Cialis order starts at 10 mg before sexual activity. That dose may go to 20 mg or down to 5 mg. Once-daily erectile dysfunction starts at 2.5 mg at about the same time each day and may go to 5 mg. BPH, and ED with BPH, sit at 5 mg once daily. The maximum recommended frequency is once per day in most patients.
08What if creatinine clearance is under 30?
Creatinine clearance under 30 mL/min, or hemodialysis, caps as-needed CIALIS at 5 mg every 72 hours. Once-daily CIALIS is not recommended in that group. Between 30 and 50 mL/min, as-needed CIALIS starts at 5 mg not more than once per day, and the maximum is 10 mg not more than once every 48 hours.
