01Do CYP3A4 inhibitors keep the US tablet on a written order?
Ketoconazole or ritonavir on the chart drops as-needed CIALIS to 10 mg not more than once every 72 hours, and once-daily CIALIS to 2.5 mg. Those cuts are another reason a US clinician still reads the list before the tablet moves.
02Why does a US clinician still sit in front of the tablet?
Organic nitrates stay off the chart with CIALIS. Regular or intermittent nitrates are a contraindication. If a nitrate later looks medically necessary in a life-threatening situation, at least 48 hours should elapse after the last CIALIS dose, and the nitrate still belongs under close monitoring.
CIALIS is not recommended after a myocardial infarction in the last 90 days, after unstable angina or angina during intercourse, after NYHA Class 2 or greater heart failure in the last 6 months, with uncontrolled arrhythmias, with hypotension under 90/50 mm Hg, with uncontrolled hypertension, or after a stroke in the last 6 months. Those groups were not in the clinical safety and efficacy trials. That list is why a US clinician still reads the chart before the tablet moves.
03What about a site that sells Cialis with no screening?
FDA warning letters go to sites that offer prescription drugs without a prescription. BeSafeRx's first check is that written order. A shop that skips it is not a state-licensed US pharmacy on that list.
A safe online pharmacy, on FDA's own wording, always requires a doctor's prescription, gives a physical US address and telephone number, keeps a licensed pharmacist on staff, and holds a state board of pharmacy license. FDA also tells people not to buy from sites that sit outside the United States.
04Is a finished actual-use trial an OTC approval?
An actual-use trial asks whether people can pick and take a labelled tablet without a clinician sitting there. Finishing that study is not the same act as FDA approving a nonprescription tadalafil tablet. The US product is still a prescription tablet until that approval exists.
On 21 January 2025, Opella, then Sanofi's consumer-healthcare arm, said FDA had lifted a clinical hold on a planned actual-use trial meant to support a prescription-to-nonprescription switch. The hold had sat since 2022 over protocol design. Lifting a hold lets that study start. It is not FDA approval of a US over-the-counter tadalafil tablet. Opella said it was still finishing the work needed to start the trial.
A US drugstore counter still asks for the written order. A big-chain pharmacy is not an over-the-counter aisle for Cialis. The bottle moves after a licensed clinician has written it.
05Does alcohol change the OTC question?
Substantial alcohol, 5 units or more on the US label, may lead to hypotension with CIALIS. That line sits with alpha-blockers and antihypertensives as a blood-pressure warning. It does not turn the US tablet into a shelf bottle.
06What did MHRA do with Cialis Together?
On 14 March 2023 the UK medicines regulator reclassified Cialis Together, tadalafil 10 mg, as a pharmacy medicine. A pharmacist discussion comes before the pack. The published limit is one tablet a day and a maximum pack of 8 tablets. Other tadalafil products in that country stay on a doctor's prescription.
MHRA said Cialis Together will not be sold to men with severe cardiovascular disorders, high cardiovascular risk, severe kidney failure, severe liver failure, or certain interacting medicines. Those groups stay under a doctor. Pharmacists received a checklist and training materials for that discussion. The notice also said erectile-dysfunction medicines are a frequent target for unlicensed and counterfeit supply.
07What if an erection lasts more than 4 hours?
An erection past 4 hours, and priapism past 6 hours, have shown up for this class. Either event, painful or not at the 4-hour mark, needs emergency care. CIALIS is used with caution in sickle cell anemia, multiple myeloma, leukemia, or anatomical deformation of the penis such as Peyronie's disease.
08Does kidney function keep Cialis off the open shelf?
Creatinine clearance under 30 mL/min, or hemodialysis, keeps as-needed CIALIS at 5 mg every 72 hours and keeps once-daily CIALIS off the chart. Between 30 and 50 mL/min, as-needed CIALIS starts at 5 mg not more than once per day. Creatinine clearance is read on a chart, not on a drugstore aisle tag.
