01Does blood DHT fall on the first day?
Finasteride produces a rapid reduction in serum DHT, reaching 65% suppression within 24 hours of oral dosing with a 1 mg tablet. Mean circulating testosterone and estradiol rose about 15% from baseline and stayed in the physiologic range. That is a serum read. Balding scalp has more DHT than hairy scalp, and the tablet lowers scalp and serum DHT. The relative contribution of those drops to the hair effect is not defined on the label. Finasteride has no affinity for the androgen receptor. A first-day DHT drop is not a first-day hair verdict. The Dosage clock still waits three months or more.
02What did investigators see as early as 3 months?
Investigator assessment used a 7-point scale for scalp hair at each visit. That assessment showed significantly greater increases in hair growth on Propecia compared with placebo as early as 3 months. Patient self-assessment of amount of hair, hair loss, and appearance also showed overall improvement compared with placebo as early as 3 months. Those are clinic scores, not a promise that you will see a photo change in week four. The self-administered questions asked about perception of growth, loss, and appearance at each clinic visit. The 7-point scale is the same family the later photograph panel used. The month-12 photo pair belongs on the stay-on ask.
03What if the mirror is unchanged at week six?
Week six is still inside the three-month-or-more window. The label does not ask you to stop because the part looks the same at day 40. Keep the daily swallow unless the clinician stops it for a reaction. Sexual symptoms that show up early are a chart call, not a reason to double the tablet. The wait for hair and the watch for reactions run at the same time. Decreased libido or an ejaculation change at week six does not mean the hair clock finished. Those are different jobs on the same bottle.
04Does a larger swallow make week four count?
Hair dosing stays at one 1 mg tablet once daily. The hair label does not write a loading dose. Taking two tablets does not buy an earlier verdict. A 5 mg tablet is a different labelled use. Keep the 1 mg line the clinician wrote. Meals do not change bioavailability, so skipping breakfast is not a shortcut either. An 80 mg daily load for three months in an overdose look did not write a faster hair clock. It is not a home experiment for week four. One tablet once daily is still the Administration sentence.
05Where does the 12-month photograph sit?
An independent panel rated standardized photographs on the same 7-point scale. That 12-month panel is a later job than the three-month Dosage clock. Numbers from that panel sit on how long you stay on finasteride, not here. Continued daily use is how the label sustains a benefit once it is there. The photograph raters were blinded. They used increases or decreases in scalp hair, not a home selfie at week six. Keep the daily 1 mg swallow through the first judgement clock before you ask for that later panel.
06What clock does the Dosage line write?
In general, daily use for three months or more is necessary before benefit is observed. That sentence sits next to one tablet (1 mg) taken once daily. It is the wait the hair label asks you to keep before you decide the swallow did nothing. Food does not shorten it. A second tablet the same day is not on the label. Meals do not change bioavailability, so skipping breakfast is not a shortcut. The finasteride shelf keeps that 1 mg once-daily line. Week six is still inside this clock, not past it.
